Home Health Licensed Practical Nurse (LPN) – New Bloomfield, PA Overview Our client is seeking a compassionate and dedicated Home Health Licensed Practical Nurse (LPN) to join their exceptional care team in New Bloomfield, PA . The ideal candidate will bring strong clinical skills, reliable professionalism, and a genuine commitment to supporting patients in their homes. This role requires an Associate degree or higher, a valid Pennsylvania LPN license, and experience working with EMR systems. Candidates should be comfortable managing their schedules independently and meeting critical documentation deadlines. Key Responsibilities Collaborate with the care team to manage and deliver individualized patient care plans Provide direct support to patients within the New Bloomfield, PA area Complete charting and documentation accurately and on time Self-manage daily schedule and travel efficiently Utilize tablets, mobile devices, and EMR software confidently (including TruChart EHR and other EMR systems) Required Qualifications Associate degree or higher 2 + years LPN experience Valid Licensed Practical Nurse (LPN) license in Pennsylvania (PA) 1+ years of experience in ANY of the following: Electronic Medical Records (EMR) Other EMR systems TruChart EHR ANY of the following valid certifications: American Heart Association Advanced Cardiac Life Support (AHA ACLS) American Heart Association Basic Life Support (AHA BLS) Schedule & Shift Details Work schedule and shift expectations will be provided during onboarding. Travel Requirements Occasional travel may be required within the local service area.
